# Burnout or Depression

Is this burnout or depression? The workplace research says the line is fragile. The autistic burnout research says the difference matters.

This is the question people arrive with most often. The honest answer depends on which burnout you mean, because the research disagrees with itself.

Depression is a diagnosis. Burnout is not, and neither is autistic burnout. That difference is where most of the confusion starts.

## What the *workplace* research found

In 2015, three researchers reviewed 92 studies comparing burnout with depression. Their conclusion was blunt.

> the distinction between burnout and depression is conceptually fragile

Fragile does not mean identical. It means the line between the two has not held up well when people try to measure it.

Maslach and Leiter treat the link as a live question for psychiatry rather than a settled one. Neither paper closes the argument.

NICE does not enter it at all. There is no NICE guideline on burnout, only NG212 on mental wellbeing at work.

## Why autistic burnout is treated *differently*

The autistic burnout research reaches the opposite conclusion. The 2020 study that defined it called it distinct from occupational burnout and from clinical depression.

A 2021 definition built with autistic experts goes further. It warns that treatment designed for depression may make autistic burnout worse.

That is a clinical stake rather than an academic one. Getting it wrong costs the person sitting in the room.

## Being *misread*

When 141 autistic adults were asked about their burnout, many had been given another explanation first. Depression, anxiety, bipolar disorder and borderline personality disorder all appeared.

None of that makes a depression diagnosis wrong. Plenty of people have both at once, and each one makes the other harder to carry.

## What we are *not* saying

We are not telling you which one you have. We do not assess anyone and we do not diagnose anyone.

We are also not offering burnout as the gentler answer. NICE publishes a full guideline on treating depression in adults. It lists talking therapies and medication among the first-line treatments.

Reaching for the word burnout to avoid the other word helps nobody. If depression is what is happening, that is the more hopeful finding of the two.

## What to bring to a *professional*

Describe what happened rather than naming it. Say when it started, what it took away and whether anything still brings relief.

Two questions separate a useful first appointment from a wasted one. Ask whether they work with burnout as well as with depression. Ask what they would do differently if the answer turned out to be the other one.
